I, (Previous Owner of the Above Animal) relinquish custody to the
adopter with the following terms:
1. Adopter agrees to provide food, water, shelter from the elements, exercise
and identification for this animal as needed for the animal's health and well
being.
2. Adopter agrees to provide veterinary care in case of illness or injury to
this animal in addition to the yearly vaccinations needed for it's continued
health.
3. Adopter agrees to not allow said animal to be used for any experimental
purposes.
4. Adopter agrees to not hold previous owner liable for any damages that the
animal may do to person or property, or any illness of the animal.
5. Adopter agrees to contact previous owner if the animal cannot remain in the
home of adopter. This animal cannot be sold or given away without express
written agreement from previous owner.
6. Previous owner agrees to take back said animal, regardless of reason, within
a reasonable amount of time when requested to do so by the adopter.
7. Adopter agrees to return said animal to the previous owner when requested to
do so because of a violation of this agreement, making no charge for it's
upkeep. If adopter does not return said animal, previous owner will take civil
action to enforce this agreement, and adopter will be responsible for the costs
of such action.
